Golf tryout physical forms

Golfers — please note you will need to make sure you get your physical forms taken care of.

Hopefully everyone is getting ready for the August tryouts.  You WILL NEED to have a copy of your EMA with you for the first day of tryouts. You cannot tryout without this.  Take care of this NOW!

Hello Chris,

Christian Wood’s physical will expire in 30 days. If your student is currently participating in a sport, another physical must be obtained before Jul 20, 2016. If your student is not currently participating in a sport, another physical must be obtained before they can tryout for one. Please print the document(s) linked below, complete the form with a doctor’s signature and turn it in to your student’s athletic administration.
